| 28,801 prizes will be awarded. Only 287,000 tickets will be sold. Chances of winning are 1 in 10.† Tickets Tickets are $100 each or a 3-Pack for $250. Guidelines for purchasing a 3-Pack for $250 are as follows: • Only ONE method of payment. • Only ONE mailing address. • Only ONE envelope will be mailed containing all THREE tickets. • All three tickets must contain the same information. Draw Dates Early Bird Ticket Sales Deadline: Midnight, October 30 Early Bird Draw Date: November 12, 2008* Final Ticket Sales Deadline: Midnight, November 20 Final Draw Date (if needed): December 3, 2008** Draw Order *In the event that all tickets are sold by the Early Bird deadline of midnight, October 30, 2008, then all draws would begin on November 7, 2008 at 9:00 a.m. The first draw would be for the Early Bird Prize. This would be followed by draws for all remaining prizes, starting with the lowest valued prize and concluding with the top valued prizes. The top 5 prize draws would be made November 12, 2008, live on Citytv’s BreakfastTelevision from 6:00 a.m. to 9:00 a.m. at 399 Maple Grove Drive, Oakville. **In the event that tickets remain after the Early Bird deadline, then only the Early Bird draw would be made on November 12, 2008 at Citytv, 299 Queen St.W.,Toronto M5V 2Z5 between 6:00 a.m. and 9:00 a.m. The final deadline for ticket sales is midnight, November 20, 2008. The remaining draws would then begin on November 28, 2008 at 9:00 a.m. starting with the lowest valued prize and concluding with the Grand Prizes. The top 5 prize draws would be made December 3, 2008, live on Citytv’s BreakfastTelevision from 6:00 a.m. to 9:00 a.m. at 399 Maple Grove Drive, Oakville. After each draw, the winner is recorded and the ticket number then goes back for the next draw. Tickets purchased before the Early Bird deadline will be eligible for all 28,801 draws. Contest Guidelines Contest guidelines prohibit the following individuals and all of their immediate family members residing in the same household from purchasing a ticket: Board members and employees of The Princess Margaret Hospital Foundation, Members of the UHN Board of Trustees and Executive Team, PMH executive team, MiraTel Solutions Inc. employees, members of licensed gaming suppliers providing services to this program. The official ticket is a receipt of purchase. It is the responsibility of ticket purchasers to ensure that they receive their ticket/s; however, purchasers do not need their ticket/s in hand in order to be included in the respective draw/s. Purchasers must be at least 18 years of age. Tickets cannot be purchased in the name of a minor. Only tickets that have been paid for and/or have been verified as paid for, are eligible to be entered for the respective draw/s. Tickets must be purchased within the province of Ontario.
